New Patient Packet

For new clients, please complete all information and bring it with you to your first visit. You may need to ask family members for history. It may seem long, but it helps us to understand a holistic view.

It is important for you to understand your rights as a client and understand office policies so we can create the best environment for your healing. Please review prior to your appointment.

Release of Information

For new or existing clients, please complete this form if you would like for Regan to release information to another source, such as a primary care physician, and fax, email, or bring it in to the office.

Frequently Asked Questions

I see clients Monday through Thursday 8am-4pm by appointment only. My last session starts at 3pm.

Counseling sessions are normally scheduled on a weekly or bi-weekly basis depending upon the nature of the concern. Sessions are typically 50 minutes long. 

I follow the professional, legal and ethical guidelines of the American Psychological Association and the state of Florida. This means that information about your counseling sessions is not shared with anyone without your expressed written permission. There are some exceptions to confidentiality: if there is the possibility of harm to the client or another person, or in cases of child or elder abuse, I am mandated to report certain information to the appropriate authorities. More information about the limits to confidentiality is written in my informed consent form and discussed in session.

Fees are $150 per 50 minute session. Payment is expected at the time of your appointment. Payment is accepted in the form of cash, check, HSA cards, or major credit cards. 

I do not accept insurance.  Most of my clients choose to not involve their insurance provider in their treatment.  Here are some of the advantages:

  • You won’t have a diagnosis label permanently attached to your health record.
  • Your records are protected, which means all information about you, including very personal information, cannot be obtained by the insurance company.
  • Your care is not dictated by the insurance company, but by the collaborative efforts formed by my professional expertise and your personal preferences and goals.

I will gladly provide you with documentation for out-of-network benefits.

Everyone’s time is valuable and appointment times are limited, so I ask that you provide 48 hours notice if you are unable to keep your scheduled appointment. This affords the opportunity to reschedule the time block should a scheduling conflict arise. If you do not cancel your appointment 48 hours before the time of your scheduled appointment, you will be billed a full session fee ($150).
